Intake form builder.Eight answers in. A ready form and auto-reply out.

Putting it to work
Google Forms
New form, then add each question in the order above with the same type. Under Settings, Responses, turn on email collection. Google Forms can email the client a copy of their answers, but it cannot send a custom reply on its own; that takes a small Apps Script step.
Microsoft Forms
New form, then add each question in the order above. A custom auto-reply needs a Power Automate flow: "When a new response is submitted", then "Send an email". Check your plan includes Power Automate first.
Form tools change their settings often. If a menu name above does not match yours, look for it under the form's Settings.
It writes the whole form

Nothing is sent
It runs in your browser. Your answers are not stored or emailed anywhere.
